1. Our Commitment to Privacy

STAT Solution Services Inc. (“STAT,” “we,” “us,” or “our”) respects your privacy and is committed to protecting the personal information entrusted to us.

STAT provides workplace health and safety services, occupational health testing, first aid and emergency support, mobile nursing, home and community care, senior wellness services, and healthcare staffing in British Columbia and Alberta.

This Privacy Policy explains how we collect, use, disclose, protect, retain, and provide access to personal information.

We handle personal information in accordance with applicable Canadian privacy legislation, including:

  • British Columbia’s Personal Information Protection Act;
  • Alberta’s Personal Information Protection Act;
  • Canada’s Personal Information Protection and Electronic Documents Act, where applicable; and
  • Other applicable health, employment, professional, and record-retention requirements.

2. Scope of This Policy

This policy applies to personal information collected through:

  • Our websites, including statsolutionservices.com and related STAT websites;
  • Telephone calls, emails, online forms, and service inquiries;
  • Home-care, nursing, wellness, first-aid, and occupational-health services;
  • Client intake and assessment processes;
  • Employment applications and staffing activities;
  • Contracts with individuals, families, employers, facilities, government agencies, and other organizations;
  • Events, worksites, mobile testing locations, and community-service locations; and
  • Any other interaction with STAT.

This policy does not apply to anonymized or aggregated information that cannot reasonably be connected to an identifiable individual.

3. What Is Personal Information?

Personal information means information about an identifiable individual. Depending on your relationship with STAT, this may include:

Contact and identity information

  • Name;
  • Address;
  • Telephone number;
  • Email address;
  • Date of birth;
  • Emergency-contact information; and
  • Government-issued identification, where reasonably required.

Health and service information

  • Health history and relevant medical information;
  • Allergies, medications, mobility needs, and support requirements;
  • Nursing assessments and care information;
  • Audiometric, pulmonary-function, mask-fit, and other occupational-health test information;
  • First-aid assessment, treatment, and incident information;
  • Workplace restrictions, accommodations, or fitness-related information;
  • Appointment and service records; and
  • Information provided by a family member, representative, healthcare professional, employer, or authorized organization.

Employment and staffing information

  • Résumés, application forms, and cover letters;
  • Education, qualifications, licences, and certifications;
  • Employment history and professional references;
  • Criminal-record or vulnerable-sector screening information, where permitted;
  • Availability, scheduling, payroll, banking, and tax information;
  • Driver’s licence and driving-record information, where relevant;
  • Workplace performance, training, safety, and incident records; and
  • Information required to evaluate eligibility for employment or placement.

Billing and business information

  • Billing address;
  • Payment and transaction information;
  • Insurance, benefits, or funding information;
  • Employer or organizational contact information;
  • Contracts, invoices, and service records; and
  • Communications with STAT.

Website and technical information

  • Internet Protocol address;
  • Browser and device information;
  • Pages visited and actions taken on our website;
  • Referring website;
  • Approximate location derived from technical information;
  • Cookie identifiers; and
  • Information submitted through online forms.

We do not intentionally collect more personal information than is reasonably required for the identified purpose.

4. How We Collect Personal Information

We may collect personal information:

  • Directly from you;
  • From a parent, guardian, substitute decision-maker, family member, or authorized representative;
  • From an employer or organization arranging a service;
  • From healthcare professionals, facilities, insurers, benefit administrators, or funding agencies, with authority or consent;
  • From references, credentialing organizations, educational institutions, professional regulators, or background-screening providers;
  • Through our websites, cookies, forms, and communications; and
  • As otherwise permitted or required by law.

When an employer arranges occupational-health testing, STAT may collect information directly from the worker. We limit information reported to the employer to what is authorized, reasonably required for the service, or permitted by law. Detailed clinical information is not ordinarily disclosed to an employer unless the worker consents or disclosure is legally authorized or required.

5. Why We Collect and Use Personal Information

We may collect and use personal information to:

  • Respond to inquiries and service requests;
  • Determine service needs and eligibility;
  • Schedule, coordinate, and provide STAT services;
  • Develop and maintain an appropriate care or support plan;
  • Deliver first aid, nursing, home support, occupational-health testing, and other health or safety services;
  • Communicate with clients, workers, family members, representatives, employers, and healthcare professionals;
  • Provide authorized service reports, test results, certificates, or invoices;
  • Coordinate referrals or emergency assistance;
  • Manage consent, safety concerns, complaints, and service quality;
  • Recruit, screen, credential, hire, schedule, and manage workers;
  • Match qualified personnel with suitable assignments;
  • Process payments, payroll, benefits, expenses, and accounts;
  • Maintain business, clinical, employment, safety, and regulatory records;
  • Meet professional, contractual, insurance, workplace-safety, and legal obligations;
  • Prevent fraud, misuse, security incidents, or unlawful activity;
  • Improve our website, services, training, and operations;
  • Send service information or marketing communications where consent is required and obtained; and
  • Carry out another purpose disclosed at or before the time of collection.

If STAT wants to use personal information for a materially different purpose, we will obtain additional consent unless the use is otherwise permitted or required by law.

6. Consent

STAT obtains consent for the collection, use, and disclosure of personal information when consent is required.

Consent may be express or implied, depending on:

  • The sensitivity of the information;
  • The reason the information is being collected;
  • The individual’s reasonable expectations; and
  • Applicable legal requirements.

Express consent may be obtained verbally, electronically, or in writing.

You may withdraw your consent, subject to legal, professional, contractual, safety, and record-retention restrictions. Withdrawing consent may affect our ability to provide a requested service.

We may collect, use, or disclose personal information without consent where permitted or required by law, including in certain emergencies, investigations, employment matters, debt-collection activities, or situations involving health and safety.

7. Children and Individuals Requiring Assistance

When a person cannot legally or meaningfully provide consent, consent may be obtained from a parent, guardian, substitute decision-maker, or other authorized representative.

We may still involve the individual in decisions about their information to the extent reasonably possible and appropriate.

Our general website is not directed to children, and children should not submit personal information through the website without the involvement of a parent or guardian.

8. When We Disclose Personal Information

STAT does not sell or rent personal information.

We may disclose personal information to:

  • STAT employees, contractors, nurses, caregivers, medics, technicians, and other authorized personnel who require it to perform their duties;
  • Healthcare professionals, emergency responders, hospitals, pharmacies, laboratories, or other care providers when authorized or reasonably necessary;
  • Family members, representatives, or substitute decision-makers with appropriate authority;
  • Employers or organizations that arranged and are authorized to receive particular occupational-health or service information;
  • Insurance companies, benefits providers, government programs, or funding agencies;
  • Professional regulators, certification bodies, workplace-safety authorities, public-health authorities, or government agencies;
  • Technology, communications, records-management, payment-processing, background-screening, accounting, legal, insurance, and other service providers;
  • A purchaser, successor, or adviser involved in a proposed or completed business transaction, subject to applicable safeguards; and
  • Other parties where you provide consent or where disclosure is permitted or required by law.

Service providers may only use personal information for the services they provide to STAT and must protect it in accordance with their agreements and applicable law.

9. Health Information and Employer-Arranged Testing

Health information is sensitive and receives a high level of protection.

When STAT provides occupational-health testing or services arranged by an employer, we will seek to explain:

  • What information is being collected;
  • Why the assessment or test is being performed;
  • What information may be reported to the employer;
  • Who will retain the detailed results; and
  • Any choices or consequences associated with the service.

Depending on the service and applicable requirements, an employer may receive information such as completion status, certification, restrictions, follow-up requirements, or another authorized conclusion. Detailed health information will only be disclosed with consent or when otherwise legally authorized or required.

Where STAT provides services on behalf of another organization, that organization may also have its own privacy policy. STAT’s handling of information may be governed by its contract with that organization and applicable law.

10. Website Cookies and Analytics

Our websites may use cookies and similar technologies to:

  • Enable website functions;
  • Remember preferences;
  • Maintain security;
  • Understand website traffic and performance;
  • Improve the visitor experience; and
  • Measure the effectiveness of communications or advertising.

Our website forms may use Google reCAPTCHA to protect against spam and misuse. Google’s privacy policy and terms may apply when that service is used.

Our website may contain links to third-party websites. STAT is not responsible for the privacy practices or content of websites that we do not operate.

11. Online Forms and Email

Information transmitted through the internet or by ordinary email cannot be guaranteed to be completely secure.

Please do not send highly sensitive medical, financial, identity, or employment information through a general website contact form or ordinary email unless STAT has specifically instructed you to do so through an approved method.

We may contact you using the contact information you provide. Commercial electronic messages will be sent in accordance with applicable Canadian anti-spam requirements. You may unsubscribe from promotional communications at any time. You may still receive necessary service, safety, billing, or administrative communications.

12. Payment Information

Payments may be processed through third-party payment providers. STAT may receive limited transaction information, while the payment provider processes payment-card information under its own privacy and security practices.

You should review the payment provider’s privacy policy before submitting payment information.

13. Storage and Processing Outside Canada

STAT and its service providers may store or process personal information in Canada or another jurisdiction.

When information is processed outside your province or outside Canada, it may be subject to the laws of that jurisdiction and may be accessible to courts, law-enforcement agencies, or government authorities under those laws.

STAT uses contractual, administrative, and technical measures appropriate to the circumstances when engaging service providers that handle personal information.

You may contact our Privacy Officer for information about our use of service providers outside Canada.

14. Protecting Personal Information

STAT uses reasonable safeguards appropriate to the sensitivity, volume, format, and use of the information. Safeguards may include:

  • Role-based access restrictions;
  • Passwords and authentication controls;
  • Secure storage and transmission methods;
  • Physical protection of paper and electronic records;
  • Confidentiality obligations;
  • Staff privacy and security training;
  • Policies governing mobile devices and remote work;
  • Vendor and contractor requirements;
  • Record-disposal procedures; and
  • Incident-response processes.

No security system is perfect. However, STAT regularly reviews its safeguards and takes reasonable steps to reduce the risk of unauthorized access, collection, use, disclosure, copying, modification, loss, or theft.

15. Privacy Breaches

STAT maintains procedures for responding to suspected or confirmed privacy breaches.

When a breach occurs, we will take reasonable steps to:

  • Contain and investigate the incident;
  • Reduce the risk of harm;
  • Document the incident;
  • Notify affected individuals when required; and
  • Report the breach to the appropriate privacy commissioner, regulator, contracting organization, or other authority when legally required.

16. Retaining and Destroying Information

STAT retains personal information only for as long as reasonably necessary to:

  • Provide services;
  • Fulfil the purposes for which it was collected;
  • Meet legal, professional, employment, tax, insurance, contractual, and regulatory requirements;
  • Resolve complaints or disputes; and
  • Protect legitimate business and legal interests.

Different records may have different retention periods.

When information is no longer required, we securely destroy, delete, or anonymize it in accordance with applicable requirements and STAT procedures.

17. Accuracy

STAT takes reasonable steps to keep personal information accurate, complete, and current where it is used to make decisions or provide services.

Please notify us if your contact, health, employment, or other relevant information changes.

18. Accessing or Correcting Your Information

Subject to applicable legal exceptions, you may request:

  • Confirmation that STAT holds personal information about you;
  • Access to your personal information;
  • Information about how your information has been used or disclosed; and
  • Correction of inaccurate or incomplete information.

Requests should be submitted in writing to the Privacy Officer. We may ask for information needed to verify your identity before responding.

In some circumstances, STAT may be unable to provide access to all requested information. For example, access may be restricted where disclosure would reveal another person’s personal information, confidential commercial information, information protected by legal privilege, or information that cannot legally be disclosed.

If access is refused, we will explain the reason where legally permitted.

19. Privacy Questions and Complaints

STAT has designated a Privacy Officer who is responsible for overseeing compliance with this policy and applicable privacy requirements.

Please do not include unnecessary medical or identity information in your initial message.

STAT will review and respond to privacy complaints in a fair and timely manner. We will not retaliate against anyone for raising a privacy concern in good faith.

If you are not satisfied with our response, you may contact the privacy commissioner responsible for your jurisdiction:

  • Office of the Information and Privacy Commissioner for British Columbia;
  • Office of the Information and Privacy Commissioner of Alberta; or
  • Office of the Privacy Commissioner of Canada.

20. Changes to This Policy

STAT may update this Privacy Policy to reflect changes in our services, technologies, legal obligations, or information-handling practices.

The current version will be posted on our website with its effective date. If a change materially affects how we handle previously collected personal information, we will provide additional notice or obtain consent where required.

21. Interpretation

If this policy conflicts with applicable legislation, the applicable legislation will govern.

Nothing in this policy limits an individual’s rights under applicable privacy law.
